New Patient Registration

Your First Step

The new patient registration link will be sent to you via email when you schedule your appointment.

If you are unable to complete the form prior to your appointment, please arrive 20 minutes early to complete it in the office.

Your First Appointment

What to bring:

  1. Insurance card(s)
  2. List of medications
  • If you are scheduled for a skin cancer check or laser treatment, do not wear make-up to your appointment or be prepared to remove your make-up.
  • Children under 18 must be accompanied by a parent or legal guardian.

Insurance Plans

  • We accept most PPOs, Tricare, Sutter HMO, Hill Physicians HMO and Medicare.  

  • We do not accept Medi-Cal

Financial Policy

  • Co-pays must be paid at the time of service.

  • Keep in mind that your insurance policy is basically a contract between you and your insurance company. You are ultimately responsible for the payment of services.

  • If we are not contracted with your insurance carrier, you are responsible for payment in full at the time of service.

  • Not all insurance plans cover all services. In the event your insurance plan determines a service to be “not covered,” you will be responsible for the complete charge.

Missed Appointment/Cancellation Policy

We kindly request a minimum of 24 hours’ notice for appointment cancellations.

  • General Appointments: Failure to attend your scheduled appointment or failure to cancel at least 24 hours in advance will result in a $50 fee.
  • Surgical & Cosmetic Appointments: Failure to attend your scheduled appointment or provide at least 24 hours’ notice for cancellations will result in a $100 fee.

Cosmetic Procedures

There is a $100 fee for cosmetic consultations with one of our providers. This fee is fully credited toward any cosmetic procedure that is completed or scheduled the day of the consultation. All cosmetic procedures require a deposit of at least $100 and payment is due in full at the time of service.  Your $100 deposit will be used if you “no-show” your appointment.

Prescription Refills

Prescription refill requests require your dermatologist to review the request before any prescription can be authorized.  Prescriptions will not be refilled if you have not been seen by one of our providers in over a year.  Check the number of refills first since many prescriptions still have refills of which you are unaware.

You may contact your pharmacy directly to request a prescription refill.  Most large pharmacies also have automated menus that you can use anytime.  They will send your refill request to us by fax or electronically.  Please allow 1-2 business days for your request to be addressed.
